Help please

I am trying to build a simple program in java. After following the instructions in the help file, I built the program as a standalone java applet. When I open the .htm file in IE, the task bar says the applet is running, but the page is blank. The java console gives an error message of unknown source. Can someone help please?

I can not get this file to work with Vitalize either. The page shows the Vitalize plugin with a black background.

The app must have crashed in Java.
Can you send the MFA to me so that I can debug it?
