The Quicktime object plays MIDI files fine and has controls for adjusting the volume within the object.

It turns out that when Windows Media Player closes, it sets the synth volume to zero. I retract my previous statement that the synth volume was at 100%. When I checked the volume while the midi file was playing in Windows Media Player, it was all the way up. I closed the Windows Master Volume component, then closed Windows Media Player and tried the sound in my game and it wouldn't work. I finally had the sense to check the volume while the game was playing and true to statements above, the volume was at 0.
My mistake, your player works just fine!

This is weird on my demo machine the synth also got set to zero.
I wonder whats doing that as I didn't set it to zero and a thought I played games with MIDI files in them before.
Something odd is going on.
As long as it's localized to the demo version, this will be harmless (for my needs). But I need to be sure the music will play on created exe's that I'll be distributing, and I suspect they will.
