Making Worm Segments?

I want to make an enemy which is a large worm with segments you can destroy individually, kinda like the one in Terraria/Death Worm.
If someone could post an example, that would be great! ( I seem to understand examples alot easier when looking through them ).

I would mainly like to know how to make an Enemy worm, but a worm that you can control ( Death worm ) would be pretty cool aswell!

Hello Keehan! Did you want the segments behind the one that you destroyed to fall off? Do you want them to follow the worm in a straight line?
