I have narrowed my issue down further:

In the past, the advice was to force direct draw mode without vram by using /ddraw /novr command line options. I removed these and tried the other modes:

standard display: significant slowdown, otherwise displays and works fine, doesn't hang on quit.
directx : works as expected, hangs on quit
directx + vram: slows to a crawl, ~5 FPS. otherwise displays and works fine, hangs on quit.
Direct3d 8: doesn't display subapplication (game breaking), breaks "subtract" visual effect on critical object. doesn't hang on quit.
Direct3d 9: doesn't display subapplication (game breaking), breaks "subtract" visual effect on critical object. doesn't hang on quit. runs fast (very nice.)

I realize now that subapplications are not recommended but it is well beyond the point where I can change this about my game. I would be happy to go back to however it was before, when I could run and quit without MMF2 hanging. I would be happiest if I could run in Direct3D 9 while keeping my subapplication (deal breaker) and subtract visual effect. As it is, no mode runs without problems whereas it used to be fine before.

My mistake- the Direct3d isn't breaking the "subtract" effect, it's just making it much more subtle. I'd be willing to work with that by adjusting my graphics, but without a working subapplication it's pointless.

Subapplication does work if I set it to "display as sprite" (again something I'd be willing to work with for the FPS boost) but then none of the activex stuff in my subapplication (list objects + cursor) displays at all. I'd hate to re-code all of this stuff with better-supported objects although the FPS boost is tempting. It'd be a major pain to replace every list object.

I have resolved my issue by following the advice I was given a while ago in this thread: http://community.clickteam.com/showthread.php?t=67691&page=10&p=504712#post504712

In short, to fix all of my problems simultaneously, I had to create a launcher application that handles changing the resolution and launching the real application. By doing this, I get full Direct3D 9, fully working subapplication, no weird hangs, the highest FPS without breaking my list objects or having to recode anything.

I mentioned it elsewhere but just for the record it seems there's a glitch with the inbuilt animations in this build, where the ending of "Launch" animation acts as "Restore Animation". More details here, including the source and exported swf.
