Koji, it's not at all a bug of MMF2.
Firstly, you must not rename a COX extension to MFX, just copy the MMF1 version of the COX file to the Extensions\mmf1 folder. COX and MFX files have different loading routines.
Secondly, if the extension is not loaded, then the problem comes from the extension, not from MMF. Verify that the 3rd party DLL required by the extension is either in the folder of the extension or in the Windows system folder. And if it's still doesn't load, then maybe this extension or its DLL is not compatible with the version of Windows you use, or depends on another DLL that can't be found on the machine.

It is exactly what I did and does not work me in build 249, but in build 248 if loads the extension.
I have proven it in Windows XP Service Pack 3 and Windows Vista BASIC and works with build 248 but not with build 249. For that reason it reports it, but it seems that to Jeff it bothers to him that these errors are reoporten.
